Commissioner of Oaths and Affidavits Services
The Municipality of North Middlesex provides Commissioner of Oaths services at the Shared Services Building, located at:

To ensure that a Commissioner is available to assist you, it is strongly encouraged to book an appointment in advance. Walk-in service may be available during regular municipal office hours; however, if you do not have an appointment, it is understood that there may not be a Commissioner available to provide the service.

Additional Information
Please note the following regarding Commissioning services:
- The document must be signed in the presence of the Commissioner, and all individuals required to sign the document must be present
- You will be required to provide valid government-issued photo identification
- Fees for Commissioner of Oaths services are established in the Municipality's Fees and Charges By-law 2026 Schedule A
- The decision to commission a document is at the sole discretion of the Commissioner
Services Provided by a Commissioner
The Commissioner of Oaths and Affidavits is authorized under the Commissioners for Taking Affidavits Act to take various types of declarations and to administer oaths, when you sign an Affidavit or Statutory Declaration.
The Commissioner may sign documents related, but not limited, to the following:
- Vehicle Transfers
- Change of Ownerships
- Insurance Claims
- Pension Plan Documents
- Consents to Travel (Deponent must provide a Government of Canada - Foreign Affairs, Trade and Development Canada recommended consent letter)
- Government related forms that specifically indicate commissioning can be completed by a Commissioner for taking Oaths and Affidavits
Services Not Provided by a Commissioner
The Commissioner will not prepare or edit affidavits, nor will they provide legal advice. Any information or clarification required for the prescribed form, should be directed to the agency or party that has requested the form. The Commissioner will not certify true copies of documents, unless given authority from the agency or party requesting the document, in writing, on the form. This excludes certification of Municipally generated documents, which can only be certified by the Director of Corporate Services / Municipal Clerk or Deputy Clerk. A Commissioner of Oaths and Affidavits is not the same as a Notary Public. A Notary Public is a person who is authorized under the Notaries Act to commission documents, and certify documents as true copies to verify signatures. If a document is required to be “notarized” as opposed to “commissioned”, the Commissioners will not commission the document.
The Commissioner will not sign documents related, but not limited, to the following:
- Documents requiring the signature of a Notary Public
- Wills, Living Wills, Codicils to Wills or Estate Settlement documents
- Divorce, Separation, Cohabitation, or Custody documents
- Real Estate documents
- Power of Attorney documents
- Documents involving debts or bankruptcy
- Forms which call upon a Commissioner, but also request certified true copies of other documents as attachments that are not issued by a government organization in Canada
- Any document that the Commissioner determines is not eligible to be commissioned pursuant to the Commissioners for Taking Affidavits Act, or where the Commissioner determines that the legal requirements for commissioning have not been met
